Mobile Spine & Movement Care
As an in-home healthcare provider, maintaining a safe, respectful, and professional environment is essential for quality care. By receiving services from Mobile Spine & Movement Care, you acknowledge and agree to the following:
Before Your First Visit
A valid photo ID and completed intake forms must be submitted prior to the initial appointment.
The provider must be informed of all individuals and animals who will be present during the visit.
Appointments are held at the residential or business address listed in your intake forms unless otherwise agreed upon in writing.
Safety Requirements
To ensure a safe and comfortable environment for both patient and provider:
A female adult must be present during all appointments involving male patients.
If this is not possible, the session may be relocated to a public or semi-public area (such as an open garage, patio, or outdoor space) or rescheduled for a time when this requirement can be met.
Right to Decline or End a Visit
The provider reserves the right to cancel, postpone, or end a visit at any time if:
The environment is unsafe
There are unexpected guests
There is harassment, inappropriate behavior, or safety concerns
The atmosphere feels uncomfortable or โoffโ
Any inappropriate, threatening, or unsafe conduct occurs
Any behavior that compromises safety will result in immediate termination of care and may be reported to the appropriate authorities if warranted.

These guidelines exist to protect the safety, dignity, and wellbeing of both the patient and the provider, ensuring every visit is professional, respectful, and conducive to effective care.
